We are also seeking to lead through innovation, by shaping both policy and practice. Our faculty have partnered with organizations such as the Carolina HealthNet, which helps provide care for the uninsured, and the I3 population Health Collaborative, a network of 27 academic primary care programs dedicated to practice improvement. Carolina HealthNet

Carolina Health Net is a collaboration between Piedmont Health Services and 大象传媒 Family Medicine to provide a system of health care for individuals in Alamance, Caswell, Chatham, Lee and Orange counties who do not have health insurance. Carolina Health Net includes affordable access to primary care which include pharmacy services, dental care, and nutrition services. For more information, please call 919-545-3440.

大象传媒 Health Alliance is a physician-led network of 大象传媒 Health Care and community providers that are transforming the way care is delivered to our patients. Our organization includes community providers, 大象传媒 Faculty Physicians, 大象传媒 Physicians Network, and providers employed by 大象传媒-owned and other providers affiliated with 大象传媒 Health Care. 大象传媒 Health Alliance, as a clinically integrated network, delivers high-quality accessible care for patients through collaboration, sharing best practices, and leveraging the collective knowledge of our providers to benefit the communities we serve.

The I3 Population Health Collaborative

I3 is a learning collaborative of 27 academic primary care programs in North Carolina, South Carolina and Virginia and includes family medicine, internal medicine, and pediatric programs. A number of our faculty members have been a part of both I3鈥檚 launch and its continuation, including Warren Newton, the former chair of 大象传媒 Family Medicine. The goal of the I3 Population Health Collaborative is to create momentum for widespread ambulatory practice improvement. This program focuses on the following changes: improve patient experience; increase access, quality and cost effectiveness of care in populations taken care of by primary care residency practices; and train primary care residents in advanced models of primary care.

North Carolina Network Consortium (NCNC)

大象传媒 Family medicine is a part of the North Carolina Network Consortium, a diverse statewide consortium of providers, academic institutions, and patients whose mission is to address pressing questions related to the delivery of primary care health services and the management of primary care problems. Seven practice-based research networks have combined resources in forming the NCNC; they represent over 2100 providers in over 350 practices across the state, including all three primary care specialties (family medicine, internal medicine, and pediatrics) and strong representation of minority populations.
