Dear families we serve,
As part of the terrific Diagnostic and Complex Care Clinic team, and since 2006, our mission is to serve children, adolescents and young adults with chronic conditions and their families. Our interdisciplinary team at the Self-management and Transition to Adulthood with Rx=treatment (STARx) Program has continued to provide new evidence in the field of healthcare transition from pediatric- to adult-focused health services. The STARx team has published over 80 scientific manuscripts in this field with new collaborators from around the country and the world. Our tools (STARx Questionnaire, TRxANSITION Index and STARx 听Passport) have been translated into 8 languages and are being used in 10 countries.
We are grateful to Dr. Carl Seashore as his persistence made it possible for the self-administered STARx Questionnaire to be now in EPIC, our electronic health record. Our collaborators at the Children鈥檚 Hospital of Orange County have our tools in the electronic health record called CERNER. Our patient education tool 鈥淎LL YOU NEED IS LOVE Syllabus鈥� is being validated and evidence of its effectiveness among USA and Mexico patients has been published in peer-reviewed journals. In collaboration with the 3C Institute, the Kidney Guide App has been created and will be deployed in several institutions across the USA.
I am also grateful for the accomplishments the International and Interdisciplinary Healthcare Transition Research Consortium has made since I co-founded it in 2007. And last, but not least, I am grateful for the work we have accomplished with Dr. Peter Kotanko and his team at the Renal Research Institute by hosting the annual International Pediatric Nephrology Fellows Conference and the PICCOLO MONDO Initiative. 听 听

Como parte del excelente equipo de la Cl铆nica de Diagn贸stico y Atenci贸n a Pacientes con Condiciones Complejas, y desde 2006, nuestra misi贸n es atender a ni帽os, adolescentes y adultos j贸venes con enfermedades cr贸nicas y sus familias. Nuestro equipo interdisciplinario del Programa de Automanejo y Transici贸n con Rx=tratamiento (STARx) ha seguido brindando nueva evidencia en el campo de servicios de salud pedi谩tricos a servicios de salud enfocados en adultos. El equipo STARx ha publicado m谩s de 80 manuscritos cient铆ficos en esta 谩rea con nuevos colaboradores en EE. UU. y el mundo. Nuestras herramientas (Cuestionario STARx, 脥ndice TRxANSITION y Pasaporte STARx) se han traducido a 8 idiomas y se utilizan en 10 pa铆ses.Agradecemos al Dr. Carl Seashore ya que su persistencia hizo posible que el Cuestionario STARx autoadministrado est茅 ahora en EPIC, nuestro registro de salud electr贸nico. Purpose
Founded in 2006, the main focus of the STARx Program is to develop and improve self-management skills and disease knowledge among adolescents and young adults with chronic health conditions. We have created the TRxANSITION Passport, TRxANSITION Index (provider verified), and the self-administered STARx Questionnaire, to identify areas of knowledge and skill deficits among patients ages 12 and older. Based on identified areas of need, education and interventions are then provided in 听an individualized, culturally, linguistically and age-appropriate fashion.

Fundado en 2006, el enfoque principal del Programa STARx es desarrollar y mejorar las habilidades de auto-manejo de la salud y el conocimiento sobre las enfermedades cr贸nicas que padecen adolescents y adultos j贸venes. Hemos creado herramientas de evaluaci贸n del auto-manejo y la preparaci贸n para la transici贸n que identifican 谩reas de conocimiento y d茅ficit de habilidades entre pacientes mayores de12 a帽os. Basados en las 谩reas de necesidad identificadas, la educaci贸n y la intervenci贸n se proporcionan de manera individualizada, cultural, lingu铆stica y apropiada para la edad.
